2.0 INCURRED TREATMENT
2.1. Outpatient treatment
2.2. Emergency cases
Emergency treatment will be given only in campus.
2.3.1 Staff and dependents can have ambulance service while on campus only.
2.3.2 UTM Health Centre is not responsible to provide any transportation to staff and dependents who wish on going to hospital for medical check-up and non-emergency referral cases.
2.4. Laboratory and X-Ray Facility
2.4.1 This facility is open at the time specified by the University.
2.4.2 University will covers staff’s examninations if being advice or recommended by doctor.
2.5. Observation ward facility
2.5.1 Staff and dependents shall be treated in ward for observation, within the clinic service time. Staff who need intensive care and observation will be refer to hospital.
2.6. Mom and baby health care facility
2.6.1 Ante-natal care and treatment during pregnancy and post-natal treatment.
a. Repeated physical check up during pregnancy.
b. Basic clinical test for urine and blood.
c. Twice ultra- sound examinations during pregnancy.
d. Medications including vitamin and mineral
2.6.2 Baby and children health
a. Periodic examinations for infants and children.
b. Immunization required by the Ministry of Health, Malaysia.
2.7. Family Planning Services
Family planning services including pills and IUCD (An Interuterine Contraceptive Device).
2.8. Immunization Services
a. Immunization that has been declared by the Ministry of Health as compulsory to be taken during outbreak.
b. Injection of tetanus vaccine for wound treatment.
c. Immunization required for travel abroad on university business only.
2.9. Dental Treatment
2.9.1 Dental treatment for staff only open hanya dibuka mengikut jadual yang ditetapkan (attachment)
2.9.2 These basic treatment is incurred by university :
a. Diagnosis and examinations
b. Abscess treatments
2.9.3 The following services will be charged a cash (attachment)
c. Endodontic treatment
d. Minor oral surgery
2.10. Non- Incurred Treatment
2.10.1 An aid to parts of the defects and disabilities such as walking sticks, wheelchairs, eyeglasses, hearing aids and so on.
2.10.2 Medical treatment and surgery for the ritual or cosmetics purposes such as circumcision and etc., except for the cases where doctors confirmed the treatment is needed to reduce the risk to patients
2.10.3 Treatment for common immunizations on their own choice.
2.10.4 Any type of vitamins, mineral and dietry supplement except for patient’s treatment.

1.0 INCURRED COSTS
1.1 Emergency treatment to saving a life which has been approved by the physician from private hospital, who give that treatment to patient; where the incident occured near private hospital.
Incurred costs are :
1.1.1 Cost for emergency treatment for emergency period until the patient condition stable enough and can be transferred to government hospital.
1.1.2 Ward payment and charged for food based on eligibility in attachment
1.2 Special treatment that apporved by specialist and Director of Health from government hospital, saying that the treatment doen not offered by the any of government hospitals.
2.0 NON- INCURRED TREATMENT
2.1. Any of hospital/ private clinic treatment that not meet item 2.0.

Universiti menanggung semua kos perkhidmatan di National Heart Institute or The Tun Hussein Onn National Eye Hospital bagi keadaan berikut :-
2.1. Consulted by medical experts of the Government Hospital
2.2. Consulted by medical experts of the Private Hospital
2.3. In emergency situation
3.0 MEDICAL LETTER OF GUARANTEE
3.1. Staff and dependent who received medical treatment at National Heart Institute (IJN) or The Tun Hussein Onn National Eye Hospital need to apply for medical letter of guarantee from the University.
4.0 NON- INCURRED TREATMENT
4.1. Medical treatment and surgery for cosmetic purposes
4.2. Treatment in National Heart Institute or The Tun Hussein Onn National Eye Hospital that not meet item 2.0.
DIALYSIS FACILITY SERVICES

1.4. Staff parents who choose SSB and still in service
2.0 INCURRED COSTS
2.1 The University will bear all charge for dialysis treatments for staff and dependents at the Government Dialysis Treatment Center and/ or University Dialysis Unit.
2.2 Dialysis treatments at private dialysis center.
2.2.1 University will not bear the dialysis treatment costs for staff at dialysis center and his/her dependents at private dialysis center except:
a. Those who lived more than 25km from University Dialysis Unit/ government dialysis treatment center.
b. Those who lived in between 25km but but the University/ government dialysis treatment unit cannot afford the treatment facilities.
2.2.2 Any drug, device or other tests done at private clinics that facilities are available in government hospitals can not be borne.
2.2.3 Treatment expenses for Continuos Ambultory Peritonial Dialisis (CAPD) including drugs, disposible equipment may be considered if not supplied by the specialist from the Government of Nephrologi.
2.2.4 Application of surgical expenses for Arterio- Venous Fistula (AVF) may be considered if referred by the government of nephrologist consultant.
2.2.5 University will incur for dialysis treatment conducted at private dialysis center which the treatment cost will not exceed RM 200.00 per treatment. Any expenses for treatment relating dialysis may be considered based on application.
3.0 NON- INCURRED TREATMENT
3.1 On their own choice.
3.2 Dialysis treatment for retired staff’s parents will not be incurred by the University.
3.3 Parents who use the staff sevices provided at UTM Health Center will be charged with specified rate for not more than 3 month from his expiration of service.
3.4 Parents of the retiree staff who wish to extend the period of validity for the services may be made thorugh an application, based on availibilty.
