1 . ELIGIBILITY

1.1 Staff ;
1.2 Husband / Wife ;
1.3 Child ;
1.4 Parents ;
1.5 Widows ;
1.6 Retirees except GSC .

2 . TREATMENT INCURRED

2.1 All treatment services at government hospitals .
2.2 Purchase of medicine endorsed by the Government Medical Officer not be supplied by the Hospital .
2.3 Purchasing, replacement and / or repair of artificial limbs and orthopedic devices certified by a Government Medical Officer but can not be supplied by the Hospital .
2.4 Expenses for installing Implant in the body for the purpose of saving lives and / or for patient comfort Physician certified by the government.
2.5 Staff and liabilities certified by a Government Medical Officer to require treatment outside the area for more than thirty -three (33 ) miles may claim an allowance entitlement , as if the staff in the due course of    official business outside the area .
The costs incurred are in accordance with the following qualifications :
a. the cost of treatment at government hospitals ;
b. expenses for an escort and certified by a Government Medical Officer except retirees
c. cost of living for patients and escorts except retirees.
2.6 Cost of travel for the patient and / or escort qualifications .
2.7 Treatment of delivery.
2.8 Expenditure for supply of wheel chairs, hearing aids.
2.9 Supply of heart treatment devices , respiratory aids , cochlear implants , colostomy bag, urine bag and the intraocular lens .
2.10 Expenditure on medical services Computed Tomography Scan ( CT Scan) and Magnetic Resonance Imaging ( MRI ) .
2:11 The cost of treatment , including hospital charges for staff and dependents are admitted to the Government Hospital as suffering from mental illness, leprosy , tuberculosis or cancer.

3 . TREATMENT IS NOT BORNE

3.1 Supply of all types / categories of tools for parents .
3.2 Treatment for disease prevention for parents .
3.3 Treatment to crowning , dentures and treatment for Corrective measure ontodontik which is a cosmetic treatment for the parents.
3.4 Medical treatment and surgical and cosmetic ritual purposes such as circumcision and the treatment of acne .
3.5 Payment of the ward.
3.6 The cost of purchasing disability aids , disability members and alternate members of tools such as crutches , eyeglasses , and dentures .
3.7 Expenditure for supply of disposable ones.

4 . WARD PAYMENT 

4.1 Staff are required to pay hospital fees for themselves and dependents . Eligibility for staff as shown in Table B / 1 .

5 . MEDICAL GUARANTEE LETTER ( GL )

5.1 Staff or dependents seeking treatment at government hospitals or dental treatment at the Government Dental Clinic , or the National Population and Family Planning (LPPKN ) or any center certified by the Medical Officer is required to submit a letter stating the name of the medical staff , office , payroll , staff number and identity card , the patient’s name and number . identity card / birth certificate of liability ( if in need of treatment is dependent ) and any selected medical centers along with a certificate from the staff to allow deductions to explain any charges .

6 . LETTER FOR MATERNITY STAFF

6.1 For female officers who give birth either in hospital or government / private are required to submit birth certificates to obtain delivery confirmation from hospital doctors . The letter can be found on the department / faculty in Annex B / 2 .

7 . MATERNITY LEAVE APPLICATION LETTER

7.1 For women who have maternity staff are required to submit a letter to the head of maternity leave / faculty in Annex B / 3 . The letter must be attached Letter Maternity confirmed the hospitals for maternity leave of sixty ( 60) days or ninety ( 90 ) days.
