1.0.  FAMILIES OF STUDENTS HEALTH SERVICES SCHEME CONDITIONS

1.1   Those eligible receiving this scheme is :

1.1.1 University families of student who already paid scheme fees for their family members for the semester.

1.2.   Those who not eligible receiving this scheme is: :

1.2.1. Families of students who withdraw.
1.2.2. Families of students who dismissed.
1.2.3. Families of students who suspended.
1.2.4. Families of students who graduated effectively starting the date of his/ her course of study final results being published.
1.2.5  Family of external candidates.

 

2.0 STUDENT’S FAMILY SERVICE SCHEME FACILITIES 

Families of students who join the scheme will receive facilities in University Health Centre as follow :

2.1.   Outpatient Clinic

Outpatient treatment is in accordance with the schedule set by the University.
The fees incurred for each treatment session is not above than rm25.00 only.

2.2   Emergency Facilities

Student’s Health Centre provides 24-hour transportation for emergency cases.

2.3   Medical Laboratory

Medical laboratory testing is conducted only on the advice or recommendations from doctor of University.

2.4.   Patient Who Need Observation

Doctor of the University can suggest any of student’s family members who need observation or treatment in observation room.

2.5.   Ambulance and Transportation Facilities

2.5.1. Emergency cases transportation
University provides transportation facilities only for emergency cases inside campus and the journey from campus to hospital.

2.5.2. Transportation expenses
University will not incur transportation expenses other than the vehicle provided by the University or Government Hospital.

 

3.0. EXPENSES EXCLUDED FROM STUDENT’S FAMILY HEALTH SERVICE SCHEME 

List of expenses that not covered by t his scheme are as follows:

3.1.   Treatment for sterility, fertility or pregnancy prevention.
3.2. Treatment and maternity-related inspections in either normal or abnormal.
3.3. Spares for parts of the body such as artificial eye, legs, and so forth.
3.4.  An aid to parts of the defects and disabilities such as walking sticks, wheelchairs, eyeglasses, hearing aids and so on.
3.5. Medical treatment and surgery for the ritual or cosmetics purposes such as circumcision and etc., except for the cases where doctors confirmed the treatment is needed to reduce the risk to patients.
3.6 Immunization treatment, include the compulsory vaccination for infants and children unless required by the Government of Malaysia in the outbreak or ATT vaccination.
3.7.   Medical check-up expenses.
3.8.   Treatment received other than treatment from UTM Health Centre
3.9.   Dental treatment.

 

4.0.   REGISTRATION & FEES

4.1     Registration

4.1.1.  This scheme registration must be made in University’s Health Centre not later than 4 weeks until semester started.
4.1.2.  Application for registration after a period in the 4.1.1  will not be accepted
4.1.3.  Application to renew registration must be made in the same period as in the 4.1.1 for each and every semester.

4.2.    Scheme fees

Fees that will be charge for joining this scheme for one semester are as follows :

4.2.1  Children age under 12 years old is RM15.00 per person.
4.2.2. Children age 12 years old and above and also adult is RM20.00 per person.

 

5.0.  GENERAL

5.1.    Any 3.0 matter which can be provided by the University Health Center for families of students will be charged at the rate set by the University.
5.2.   Families of students who did not participate in this scheme can have access to treatment at University Health Centre for a fee set by the University.
5.3.   Families of students should bring along member card during their treatment in University Health Centre.
